So, while there may be three pips shown on the rank bar, only two of them (the white pips) actually help you progress through the ranks. The black pip is simply a buffer to prevent you from getting demoted.
In my personal experience with DBD, I have encountered situations where I managed to double pip (earning two white pips) in a match due to exceptional performance. However, I have never come across a scenario where triple pipping was possible. The ranking system is designed in a way that limits players to double pipping at most.
